Certification of medical records by the Ministry of Healthcare of Ukraine

Ukrainian medical records of required form may be legalized by the Ministry of Healthcare of Ukraine. These records must be duly executed and certified by an oblast Department of Healthcare of appropriate regional jurisdiction. Thus, for example, records issued in the Odesa Oblast must be certified by the Department of Healthcare of the Odesa Oblast.

In Kyiv, medical records of required form are issued by the Central City Clinical Hospital located at the following address: Shovkovychna Str. 39/1, tel: 234–73–64. These records are valid during 6 months from the issue date.
